Liability claim must be submitted with the following supporting documentation:

Completed and signed Liability Incident Report Form
Letter from third party advising why they are holding our insured liable for loss, damage, injury ect.

Letter from our insured giving reasons why they feel they were negligent - if applicable (advise clients that they must not to accept liability under any circumstance).
